Lost/Damaged or Stolen Passport

You are unable to locate your current valid passport.

Your passport has been stolen or damaged, and it is still valid.
Step 1 Gather and Prepare Following Required Documents
1. Completed Application Form DS-11:
Click here to complete Form DS-11 Online: Following the instruction to answer all questions online. After answered all questions, you will see a “CREATE FORM” at bottom of the page. Click the button, then form DS-11 will come up. Please print out the form DS-11.
After printing out the form, do not sign this form until you are instructed to do so by the acceptance agent.
2. Completed DS-64 form (Statement regarding lost/stolen passport)
This form should be submitted along with the DS-11 Application. Click here to fill out and print out the DS-64 form
3. Two Passport-type photos are required.

Photos must meet the government specifications photo requirements. Must be official passport-type only and approximately 2″ x 2″ with a clear background. The photo must be in color. Do not wear a hat, glasses, or any item that covers the face.

Homemade Digital Photographs are not acceptable.
4. Proof of Identification: please provide one of following

State issued driver’s license (at least 6 months old) bearing a photo or
-
Military I. D. bearing a photo.
5. Proof of Citizenship: You need to provide one of the following:

Your certified birth certificate (Need help to get a birth certificate, please click here) or
-
Naturalization papers
6. Proof of Departure: Please provide one of the following items

A copy of your airplane ticket or
-
A copy of your itinerary from the travel agent or airline
7. Letter of Authorization
Click here to download and complete the Authorization Letter. This letter gives BCV the authority to submit your application and pick up your new passport on your behalf. Please complete and print out and singed two copies of Authorization Letter.
8. Processing Fees: Including U.S government fee (A) and BCV service fee (B)
A. U.S. Government Fee:
Passport Book only: $209.53 (including return shipping fee)
Passport Book and Passport Card: $239.53
Make one check or money order payable to “U.S. Department of State” (this is the government processing fee).

Write applicant’s full name and date of birth on the check or money order.

All personal checks must show a printed address.

Do not cross out and rewrite any information on the check.

Temporary or starter check is not acceptable.

10. Order Form

Step 2 Get Your Application Authorized
You need to have your applications authorized by local government officials called Acceptance Agents (AA).

Bring all the documents listed above to visit an Acceptance Agent. Click here to find an Acceptance Agent nearest you.​

Please Inform the Acceptance Agency that you are using an expediting service to “hand Carry” your application to the passport agency for speeding processing.

After signing the application form DS-11 in front of the Acceptance Agency, the Agency should sign the form and must seal-up the following items in an envelope and marked “to be opened by U.S. Passport Office only.”

Executed Form DS-11

Two identical passport photos
-
Proof of citizenship
-
Check or Money order pay to “US Dept. of State”

One copy of the proof departure
-
The letter of authorization
-
A copy of your driver license.

Seven items mentioned above must be sealed inside envelope. If missing any one, Passport Agency will not process your application
Note

Please write the 1) Applicant Name, 2) Date of birth, 3) Departure date on the outside of the sealed envelope

Do not seal the payment to BCV inside the sealed envelope.

Do not open this sealed envelope.

Important: Make sure the agent returns the sealed envelope to you. If the agent does not feel at ease about releasing the envelope, suggest that he/she refer to page 31 or 44 of their Passport Agent’s Manual, “Hand-carrying of Executed Applications”.
Step 3: Ship Documents to BCV
