Child Passport Application
Child Passport Application Process is for those applicants in the following:
-
You are applying for a first time U.S. passport for minors under age 16; OR
-
You are renewing a U.S. passport for minors under age 16.
Step 1 Gather and Prepare Following Required Documents
1. Completed Application Form DS-11:
Click here to complete Form DS-11 Online: Following the instruction to answer all questions online. After answered all questions, you will see a “CREATE FORM” at bottom of the page. Click the button, then form DS-11 will come up. Please print out the form DS-11.

After printing out the form, do not sign this form until you are instructed to do so by the acceptance agent.
2. Two Passport-type photos are required.

Photos must meet the government specifications photo requirements. Must be official passport-type only and approximately 2″ x 2″ with a clear background. The photo must be in color. Do not wear a hat, glasses, or any item that covers the face.

Homemade Digital Photographs are not acceptable.
3. Proof of child's Citizenship: You need to provide one of the following:

Child birth Certificate (Need help to get a birth certificate, please click here,

Child naturalization papers
-
Child's old passport.
4. Parent Permission (if necessary):
For children passport application, both parents should appear before the Acceptance Agent or providing the following:
-
Applying parent submits second parent's notarized Form DS-3053. Statement of consent, or other notarized written statement consenting to passport issuance for child or
-
Applying parent submits primary evidence of sole authority to apply, such as one of the following:
-
Child's certified U.S. or foreign birth certificate listing only applying parent
-
Report of birth abroad (FS-240) or certification of birth abroad (DS13-50) listing only one parent
-
Court order granting sole custody
-
Adoption decree( listing only the applying parent)
-
Court order specifically permitting applying parent's travel with the child
-
Judicial declaration of incompetence of non-applying parent
-
Death certificate of non-applying parent
-
5. Proof of Parent's Identity:
Copy of applying parent's driver's license issued more than six months ago (Not temporary or learner's license).
6. Proof of Departure: Please provide one of the following items

A copy of your airplane ticket or
-
A copy of your itinerary from the travel agent or airline
7. Letter of Authorization
Click here to download and complete the Authorization Letter. This letter gives BCV the authority to submit your application and pick up your new passport on your behalf. Please complete and print out two copies of Authorization Letter.
8. Processing Fees and Processing Time
A. U.S. Government Fee:
Passport Book only: $179.53
Passport Book and Passport Card: $197.53

Write applicant’s full name and date of birth on the check or money order.

All personal checks must show a printed address.

Do not cross out and rewrite any information on the check.

Temporary or starter check is not acceptable.
B. Processing Time:
-
3-5 business days processing

6-9 Business days processing
-
10-15 Business days processing.

Step 2 Get Your Application Authorized
Bring all the documents listed above to visit an Acceptance Agent. Click here to find an Acceptance Agent nearest you.
Applicants under 16 are required to appear in person with both parents before the Acceptance Agent. If only one parent can appear, the parent must provide the permission or Statement of Consent from second parent ( No. 4 and 5 in step 1)

​Please Inform the Acceptance Agency that you are using an expediting service to “hand Carry” your application to the passport agency for speeding processing.

After signing the application form DS-11 in front of the Acceptance Agency, the Agency should sign the form and must seal-up the following items in an envelope and marked “to be opened by U.S. Passport Office only.”

Executed Form DS-11

Two identical passport photos
-
Proof of citizenship
-
Check or Money order pay to “US Dept. of State” ($179.53 for passport book only, $197.53 for book and card)

One copy of the proof departure
-
The letter of authorization
-
Proof of Parent's Identity
-
Seven items mentioned above must be sealed inside envelope. If missing any one, Passport Agency will not process your application
Note

Please write the 1) Applicant Name, 2) Date of birth, 3) Departure date on the outside of the sealed envelope

Do not seal the payment to BCV inside the sealed envelope.

Do not open this sealed envelope.

Important: Make sure the agent returns the sealed envelope to you. If the agent does not feel at ease about releasing the envelope, suggest that he/she refer to page 31 or 44 of their Passport Agent’s Manual, “Hand-carrying of Executed Applications”.
