2. Completed DS-5504 Form
Reminder:
- When completing DS-11 Form online, please fill in our address ( 1502 Rosalie St, Houston, TX 77004) in the block "
where should the passport be mailed ? ", then answer " No" in the section " Is this your permanent address", then
filling in your permanent address and phone number.
- If you don't have any travel plan, or your departure date is not within 14 days, please make sure do not fill in the travel
plan section online and leave it blank.
- No alterations can be made to the original signature and date on the government application. Example: White out ect.
Any changes made to the original signature or date will result in the need to re-execute a new application
- The State Department states that the application (DS-5504) has to be submitted within 5 days of when it is dated.
Please date the application for the day BCV will receive it.

3. Passport Photos
Send us two passport photos. Photos must meet the government specifications, see photo requirements here. We
strongly recommend not sending digital photo
|
4. Proof of Name Change:
You must submit one of the following
- A certified marriage certificate,
- Divorce decree or
- court order showing the change of name.
The name change document must bear the official seal of the issuing authority.

5. Proof of Departure:
Submit one of the following:
Travel itinerary or
Your ticket from an airline or travel agency

6. Letter of Authorization:
Click here to download and complete the Authorization Letter. This letter gives BCV the authority to submit your application and
pick up your new passport on your behalf. (Pleasemake one extra copy)

7. Fee and
Payment

A. Government Fee: One check or money order made payable to "U.S. Department of State" for $60
(Write the applicant's full name and date of birth on the check or money order).

B. BCV Service Fee: Use the table below to select the service you need and the corresponding BCV
service fee. Make your check or money order payable to BCV
