Collect and Send the Following Documents
1.  Valid Passport
    You must send your valid actual passport to us.  Your passport must have at least 6 months validity remaining
    before expiration  and must have at least one blank visa page.
    Note: The last three pages on the U.S. Passport are not visa pages and are not acceptable. If your passport doesn't
    meet the requirement, click here to see our passport services.
  • Non-U.S. Citizens must send original green cards, or copy of  "H" visa or "F" visa.

2.   Application Form
  • Click here to download Form V.2011A that must be completely filled on computer. One form per applicant with one
    photo. Do not leave any areas blank but instead write "N/A".
  • Non-US citizens must also fill out a supplementary application  Form V. 2011B  
  • Note: The photo with the application form must be passport style that is professionally taken and printed.

    Make one check or money order payable to:  BCV Visa and Passport Services. Credit  card are accepted with 3%
    surcharge.

Shipping of Application
Please send the following documents to BCV via Fedex  UPS or other security mail:
  1. Your original actual passport
  2. One application Form with passport style photo for each applicant
  3. Your check or money order payable to BCV
  4. Other required documents ( if necessary, such as Green Card Copy)
For same day service, the application must be received before 8:30 AM, CST.
