Documents Required for Crew Visa Application
1. Valid Passport
    You must send your valid actual passport to us.  Your passport must not expire within 6 months and must
    have at least one blank visa page.
  • Non-U.S. citizens must send a copy of their green cards or valid U.S. visas.
2. Application Form
    Click here to download the form.  After filling the form online, print and sign the completed application form
    for submission.
3. Photo
    One passport photo, which must be 2x2, black and white, or color, with a white, or light background, and
    show a full, front view of your face.
4. Crew Member ID
    Copy of your crew Member ID
5. Business Letter
    A letter from the airline to the Chinese Consulate stating your purpose of visit.
6. BCV Visa Order Form
    Complete the BCV Visa Order Form online. This online form is essential for us to back your visa in a correct
    address and update your order status via email.
