Collect and Send the Following Documents

1. Valid Passport
You must send your valid actual passport to us. Your passport must have at least 6 months validity
remaining before expiration and must have at least one blank visa page.
Note: The last three pages on the U.S. Passport are not visa pages and are not acceptable. If your passport
doesn't meet the requirement, click here to see our passport services.
- Non-U.S. citizens must send original green cards or valid U.S. visa.
2. Application Form
- Click here to download the Form V.2011A that must be completely filled on computer. One form per
applicant with one photo. Do not leave any areas blank but instead write "N/A".
- Non-US citizens must also fill out a supplementary application Form V. 2011B
Note: The photo with the application form must be passport style that is professionally taken and printed.
3. Crew Member ID
Copy of your crew Member ID
4. Business Letter
A letter from the airline to the Chinese Consulate stating your purpose of visit.
5. BCV Visa Order Form
6. Payment:
Make one check payable to: BCV Visa and Passport Services (Checks, cashier checks, money orders or
credit card are accepted).

Please send all the documents mentioned above with your check or money order payable to BCV (FedEx,
UPS, or USPS Express recommended). Multiple applications can be sent and returned in one package.

therefore use the services of FedEx Priority, UPS or DHL only.
