Hawaii. Saturday delivery is not included.

Enclosing a prepaid airbill (FedEx, UPS), or an envelope with sufficient stamps, including
Post Office Express Mail, Priority Mail with Delivery Confirmation, or Certified Mail. Regular
Mails or Priority Mails without delivery confirmation are not accepted.

Prepare, Collect and Send the Following Documents
|
|
1. Valid Passport
You must send your valid actual passport to us. Your passport must not expire within 6 months and must have at
least one blank visa page.
- Non-U.S. citizens must send a copy of their green cards, "H" visa or "F" visa.
- First time applicants who were previously Chinese citizens must submit the data page (first page) of your China
passport in addition to your US passport.
2. Application Form
Click here to download the form. Completed online or handwrite, with one passport photo.
Note: The photo with the application form must be passport style that is professionally taken and printed.
3. Business letter
Business Invitation letter from the company or any organization in China, or business letter from your own
company on your company letterhead.

Shipping of Application
Please send the following documents to BCV via Fedex UPS or other security mail:
- Your original actual passport
- One application Form with passport style photo for each applicants
- Your business Letter
- Your check or money order payable to BCV
- Other required documents ( if necessary, such as Green Card Copy)
For same day service, the application must be received before 10:30 AM, CST.
